Centennial Adultcare center is a proud provider and partner with the Department of Veterans Affairs. TheVA provides assistance to retired veterans and anyone receiving veteran’s benefits may be eligible for Home and Community Based Services (HCBS) including Adult Day Health Care, and In-Home Services (Homemaker and Home Health Aide Services) at little or no additional costs. Covered benefits of Adult Day Health Care through the VA include round-trip transportation and personal care services. HCBS Services must be ordered by a VA physician and authorized through the VA. Greater Nashville Regional Council, Area Agency on Aging and Disability (GNRC-AAAD)

GNRC is a regional agency that plans programs and services as advocates for the elder population and adults with disabilities. It serves as the entry point for TennCare and the distribution point for information and assistance on services for adults, including all of Tennessee’s Home and Community Based Programs. They are the single contact and entry point for the Medicaid/TennCare CHOICES program, and administer other financial assistance programs to aid adults in need of day care services, and in-home services (State of Tennessee OPTIONS Program). TennCare (Medicaid) CHOICES

TennCare CHOICES in Long-Term Care, or CHOICES, is a state run Medicad program designed to provide long-term support and services to adults age 65 and older, or 21 and older with disabilities, who meet the low-income, and asset requirements. Long-Term care includes assistance with activities of daily living which can be provided at home, in an assisted living facility, in an adult day health care facility, or in a skilled nursing facility. Services designed to allow individuals to remain in their own homes or in the community are called Home and Community Based Services (HCBS) and include CHOICES groups 2 and 3 enrollees. Benefits and services of the CHOICES program are administered by any of three managed care organizations (MCOs), United Health Care, Amerigroup, or Blue Cross-Blue Shield Tennessee.

Long-Term Care Insurance

Centennial Adultcare Center accepts Long-term care insurance and will happily work directly with providersto assist with authorizations and payments for covered services. In some cases the insurance provider will reimburse Centennial Adultcare Center, or in other cases individuals may be reimbursed directly and can remit payment for services themselves. Please contact your insurance provider or representative listed on your policy to inquire about covered Adult Day Health Care or In-Home Care Services.

Transportation Services

Multiple options are available for assistance with transportation services. Centennial Adultcare Center offers daily transportation throughout Davidson County as well as to and from Murfreesboro. These services are provided at no cost for VA members and at a daily cost for others. We also work very closely with the Metro Transit Authority (MTA) and Access Ride in order to provide transportation to members who cannot afford to pay privately. In some cases, financial assistance may be available directly from Centennial Adultcare Center in order to provide transportation to low-income members who qualify.
